The Instant Form
What this is for
The form is the landing page. There is no landing page. A patient taps the ad, the form opens inside Facebook or Instagram, they fill in four fields, done. Every lead lands in GoHighLevel the same second.
The rules
- Four fields only: name, phone, email, best time to call. Every extra question costs you leads.
- Form type: start with "more volume." Switch to "higher intent" later if you want fewer, better leads.
- Privacy policy link: required by Meta. Use your website's privacy page.
- Thank-you screen: "Watch your phone. A text with your booking link is on its way." Nothing else. No phone number to call, no website link.
Do this
- Go to business.facebook.com, open your Page, and find Instant Forms (under Leads or Publishing tools, depending on the year). Meta's own steps: Create an Instant Form. Their screens change. If ours looks different, follow theirs.
- Name the form after the offer: "Couples heart scan."
- Add the four fields. Remove anything Meta added on its own.
- Paste your privacy policy link.
- Write the thank-you screen text above.
- Save.
Connect it to GoHighLevel
Inside GoHighLevel: Integrations, then Facebook, connect the Page, pick the form, map the four fields. The exact clicks are in the module 2.2 video.
Then use Meta's test-lead tool to send a fake lead. Watch it appear in Contacts. If it does not appear, it is almost always the field mapping. Check the phone field first.
Test it with your own phone
Preview the ad, submit the form from your own phone, and wait for the text. This is the only test that counts. If you got the text, a patient will.
Done when
You submitted the form on your own phone and got the booking text.
